Overview

Thanks for your interest in our free Blood Pressure Wellness Program! We’re excited to work with you to lower your blood pressure and risk of heart disease.

The goal of the program is to help you build healthy lifestyle habits so you can reach (and maintain) a blood pressure of 140/90 or less. The program will complement any medications that your doctor or nurse practitioner might prescribe.

Program benefits

Big picture: You’ll get hands-on support taking steps to lower your blood pressure and risk of heart disease.

As part of the free program, you’ll also get:

  • A free blood pressure cuff (if you don’t have one already)
  • Training on how to check and track your blood pressure on your own
  • Coaching and education to help you build healthy lifestyle habits
  • A $25 gift card for completing the program

What to expect

The Blood Pressure Wellness Program includes 5 sessions with a health guide. Your sessions will happen over 2 to 3 months.

Each session will cover a different topic related to lowering blood pressure:

Program overview & blood pressure basics

Nutrition

Physical activity

Stress management

Sleep

What we need from you

  • Participate in all 5 sessions with your health guide.
  • Review the resources in your care plan before each session.
  • Complete the “on your own” home activities we give you.
  • Bring questions and be ready to actively participate.
  • Take a survey before and after the program.
  • Check your blood pressure at least twice a month.
